Due to sudden and unforeseen circumstances, the Grassroots Dental Department has been forced to vacate our previous facility in Powder Springs, GA.

However, if you would like to be notified when we have appointments available, please complete and submit the Registration form to the right. After registering, you will immediately be taken to our short online application. Once your application is complete, you will be added to the list to contact once we find a new location!

 If you are having an emergency or need services quickly, we advise you to look for another dental provider who has openings available, as we will be unable to offer you an appointment until we find a new office location.

  1. Fill out the form to the right. Once you have submitted your Registration, you will then be taken to a NEW page to start your online Dental Services Application.

  2. If you are registering for a new patient that is NOT yourself, you MUST submit the registration form with THE PATIENT's name and contact information.

Every visit at the Community Health Center requires a prepayment in order to schedule. The amount of the prepayment varies for different services and it is applied to the total cost of the service being rendered. We require a prepayemnt to schedule because that secures your appointment to see the doctor. Our time, like yours, is valuable and requiring a prepayment to make an appointment helps us to keep our schedule full and efficient and keeps our prices low, which is good for you as the patient and for the community. Your prepayment will be collected when we call you to schedule your appointment.
